PCOS and Fertility: A Clear First Plan

Couple discussing PCOS and fertility with a gynecologist at Samanwi Clinics, Hyderabad

Polycystic ovary syndrome (PCOS) can make periods irregular and ovulation unpredictable, but most women do not start with IVF. A first fertility visit looks at cycles, weight, thyroid and sugar tests when needed, and ultrasound. Treatment often begins with lifestyle steps and ovulation support. Dr. Swathi Srirangam offers that work-up at Samanwi Clinics in Saroornagar, Hyderabad.

What is PCOS, and what is it not?

PCOS is a hormone pattern. Doctors usually look for a mix of irregular periods, signs of higher androgens such as acne or excess hair, and polycystic-appearing ovaries on ultrasound. You do not need every feature to be assessed, and an ultrasound report that says “polycystic ovaries” is not the same as a full diagnosis.

PCOS is also not a statement that pregnancy is impossible. Many women with PCOS release an egg with the right support, and some conceive without tablets once cycles become more regular.

How does PCOS affect trying for a baby?

Pregnancy needs an egg to be released. If periods come every few months, ovulation may be infrequent, so timing intercourse around a calendar app is less reliable. Painful or very heavy periods, and long gaps with no bleed, should be discussed rather than normalised. Those symptoms are covered on the clinic’s menstrual disorders page as well.

Other causes of irregular cycles, including thyroid disease, high prolactin, and pregnancy itself, are checked before a PCOS label is used to plan fertility treatment.

What does the first visit usually include?

At Samanwi Clinics the first plan is built from history and a few targeted tests, not a package of every scan:

  • Cycle length, previous pregnancies, and how long you have been trying
  • Weight, blood pressure, and signs such as acne or unwanted hair
  • Selected blood tests, often including thyroid and glucose measures
  • A pelvic ultrasound timed to the cycle when that helps
  • A plan for the male partner, because fertility is a couple’s evaluation

When is IVF discussed?

Ovulation induction, sometimes followed by intrauterine insemination (IUI), is commonly discussed before in vitro fertilisation. IVF or ICSI becomes relevant if tubes are blocked, sperm parameters are significantly low, simpler treatment has not worked, or age makes a faster route wiser. Those procedures are described on the IVF procedures page.

Couples travel from LB Nagar, Dilsukhnagar, Nagole, and Malakpet for this counselling. The goal of the first hour is a sequence you can follow, with review dates, not a promise of a month in which pregnancy will occur.

Common questions

Can I get pregnant with PCOS without IVF?

Many women do. Restoring more predictable ovulation is the usual first goal. IVF is reserved for specific findings or when simpler treatment has not led to pregnancy.

Do polycystic ovaries on ultrasound always mean PCOS?

No. The scan is one part of the picture. Irregular cycles and clinical or blood signs of higher androgens matter, and other hormone problems must be ruled out.

Should my partner be tested too?

Yes. A semen analysis is a standard early test. Samanwi Clinics can arrange male fertility assessment with Dr. Aman Chandra Deshpande.

How long should we try before a PCOS fertility visit?

If cycles are very irregular, book earlier. Otherwise, seek care after 12 months of trying, or after 6 months if the woman is 35 or older.
